Program summary

This 20-25 week, SkillBridge program is designed to immerse a transitioning service member in the role of a Patient Care Coordinator within the dynamic field of sleep medicine. The participant will become an integral part of our team, responsible for guiding patients through the entire treatment process for sleep apnea using advanced oral appliance therapy. It can be offered fully remote, fully in-person in Nevada, or hybrid (remote and in-person).The program provides a structured curriculum that begins with foundational knowledge, covering topics such as the science of sleep apnea, our innovative treatment solutions, the competitive landscape, and essential administrative functions like patient scheduling, insurance verification, and HIPAA compliance.Following the initial learning phase, the participant will transition to hands-on, practical experience. Through a combination of shadowing experienced mentors, supervised practice in live environments, and observational learning, the service member will master core competencies in patient communication, care coordination, and collaboration with clinical staff. The training also includes exposure to the durable medical equipment (DME) sales cycle and physician outreach strategies.The program culminates with knowledge assessments and a final capstone presentation, ensuring the participant is fully prepared to transition into a successful civilian career in healthcare administration and patient services.

Roles & job description

Patient Care CoordinatorPatient Experience & Communication•Serve as the primary point of contact for patients seeking treatment for sleep apnea with dental appliances.•Provide education on the treatment process, appointment expectations, and next steps in care.•Answer patient inquiries via phone, email, and in person in a professional and timely manner.•Maintain a welcoming, patient-focused environment that reflects the organization’s commitment to quality care.Scheduling & Care Coordination•Schedule patient consultations, appliance fittings, follow-up visits, and sleep study coordination as needed.•Coordinate appointments with dentists, sleep physicians, and clinical staff.•Ensure appropriate documentation, referrals, and diagnostic information are received prior to appointments.•Manage provider schedules to maximize efficiency and patient access.Insurance & Financial Coordination•Verify insurance eligibility and benefits related to sleep apnea treatment and oral appliance therapy.•Assist with pre-authorizations and required documentation.•Communicate estimated patient responsibility and available financing options when applicable.•Collect co-payments, deductibles, and patient payments in accordance with company policy.Administrative Duties•Maintain accurate patient records in the practice management system.•Scan, upload, and document patient records including referrals, sleep studies, and clinical documentation.•Ensure compliance with HIPAA and patient privacy standards.•Assist with daily office operations including patient check-in/check-out and phone management.Collaboration•Work closely with clinical staff to ensure seamless patient flow.•Communicate with referring physicians, dental providers, and external partners as needed.•Support the clinic team in maintaining efficient workflows and patient satisfaction.
