Patient Services and Care Coordination
The Patient Centered Services, LLC SkillBridge Fellowship is a structured, non-paid, experiential learning program designed to prepare transitioning Service Members for civilian careers supporting CMS- and VA-aligned healthcare administration, patient services, and care coordination functions. All activities are training-focused, supervised, educational in nature, and do not displace paid employees.
- Understand CMS and VA healthcare delivery frameworks- Perform healthcare administrative functions under supervision- Support patient intake, scheduling, and referral processes- Assist with CMS and VA documentation and compliance requirements- Communicate effectively with patients, providers, and care teams- Support care coordination and patient navigation activities- Demonstrate readiness for civilian healthcare employment
